Connect the Dots is a user pays service. Our fees are paid by the people seeking care or their families, and we do not receive fees, commissions or other payments from aged care providers or other service providers. Services provided to clients are governed by the terms and conditions in the proposal provided to each client. These Terms of Use apply to use of the website only.

5. Links to other websites
This website may link to websites operated by aged care providers, government agencies and other third parties. We do not control and are not responsible for the content or privacy practices of those websites.

6. Reviews and testimonials
Reviews displayed on this website are sourced from Google and reflect the experiences of individual clients. Individual outcomes may vary.

The content on this website, including text, graphics, logos and resources, is owned by or licensed to CTD.NET.AU Pty Ltd. You may view, download and print content for your own personal, non-commercial use. You must not otherwise reproduce, distribute or use the content without our written permission.

To provide our services, we may collect:

• Contact details, such as names, phone numbers, email and postal addresses
• Information about the person requiring care, including age, care needs, current living situation and preferred locations
• Health information, such as aged care assessment outcomes, medical and clinical information, medication details and GP letters
• Information about legal arrangements, such as Enduring Power of Attorney documents
• Financial information required to complete Services Australia means testing forms, where you engage us for that service
• Billing and payment details
• Website usage information, collected through cookies and analytics tools

Health information is sensitive information under the Privacy Act. We only collect it with consent and where it is reasonably necessary to provide our services.

3. How we collect information
We collect information directly from you when you contact us, complete an enquiry form, speak with our team or engage our services. With your consent, we may also collect information from hospitals, health professionals, aged care providers and family members or representatives involved in the care of the person requiring care.

4. Collecting information on behalf of another person
Where you provide information about another person, such as a family member requiring care, you confirm that you are authorised to do so and that the person (or their legal representative) is aware of and consents to the information being provided.

5. How we use information
We use personal information to:

• Respond to enquiries and provide our placement services
• Identify suitable aged care providers and arrange tours
• Share relevant information with aged care providers to support admission
• Complete forms you have engaged us to complete
• Issue invoices and manage payments
• Send updates and information about our services (you can opt out at any time)
• Improve our website and services

6. Disclosure of information
We may disclose personal information to:

• Aged care providers, for the purpose of assessing and arranging placement
• Hospitals and health professionals involved in the person’s care
• Services Australia, where you engage us to complete means testing forms
• Other service providers you ask us to connect you with
• Our service providers, such as IT, CRM, hosting and payment providers, who assist us in operating our business
• Anyone else where required or authorised by law

We do not sell personal information.

7. Where information is stored
Client and enquiry information is stored in our CRM, which is hosted in Australia. Some website analytics and advertising tools we use, such as Google and Meta, may process information on servers located outside Australia, including in the United States. We take reasonable steps to ensure these providers handle information in a manner consistent with the Australian Privacy Principles.

8. Data security
We take reasonable steps to protect personal information from misuse, interference, loss and unauthorised access, modification or disclosure. Information is stored on secure systems with access limited to staff who need it to provide our services. We retain information only for as long as needed for our services and legal obligations, after which it is securely destroyed or de-identified.

9. Cookies, analytics and advertising
Our website uses cookies and similar technologies, including Google Analytics, Google Ads, Google Tag Manager, Meta Pixel and HubSpot. These help us understand how visitors use our site, measure the effectiveness of our advertising and show relevant ads on other platforms.
